Out of Area Patient Registration
Thank you for choosing to Re-Register at our practice.
As an existing patient registered at our Practice who has moved out of our catchment area but still reside within Hackney we will re-register you as an Out of Area Patient Registration Scheme we would like to inform you of the following important points:
Limited Services
As an out-of-area patient you will have access to general GP services such as:
- Routine appointments
- Repeat prescriptions
- Referrals to hospitals or specialist care
However some services are not available under this arrangement.
No Home Visits
We do not offer home visits to patients registered out of area.
If you require urgent medical attention at home you will need to contact:
- NHS 111 for advice
- Or attend the nearest Urgent Care Centre / A&E if necessary
Emergency or Urgent Care While at Home
In case of urgent medical needs when you are at your home address (outside our boundary), you may:
- Be advised to contact NHS 111
- Be referred to a local provider near your home for treatment
Suitability and Safety
The practice will assess whether your registration is clinically appropriate and practical. If it is not, we may decline your registration or discuss alternative care arrangements.
Onward Referrals and Services
If you are referred to as community services or hospital care these will usually be near the practice not your home. You are responsible for arranging your own transport for appointments and follow-ups, however you can check with Reception if you require assistance.
Communication
You must ensure we have up-to-date contact information (phone/email), as postal communication may not be effective due to the distance. Please let us know your preferred methos of communication.
Your Consent
By registering as an Out of Area Patient, you confirm that you:
- Understand and accept the limitations of this registration
- Are willing and able to travel to the practice for all routine and urgent care
- Do not expect home visits or local care from the practice.
