Self Zone


From 1st June 2024, Nightingale Practice and Lower Clapton General Practice have joined Statham Grove Surgery in the LARC self-referral pilot to improve access to contraceptive services in City and Hackney by allowing patients to self-refer for implant and coil fittings or removal.

Self-referral process and where to access the self-referral forms:

  • Posters will be sent out to practices which have a QR referral code which directs patients to the self-referral forms. We ask practices to display these posters in the waiting rooms, WCs etc.
  • Practices can share the self-referral link with patients (see the information sheet attached below)
  • Paper self-referral forms have been produced for patients who do not have digital access. Practices can make these available at reception. We attach a paper form here for your convenience.

Referrals will be received by the clinicians who will schedule a telephonic appointment with the patient. Patients will be asked to complete a pre-assessment questionnaire and once they meet the criteria, they will be able to book an appointment online or be booked into the next available slot of their choosing.
